Employment Opportunity: Community Programs Navigator

STATUS:              Non-Exempt

REPORTS TO:     Aging in Place Coordinator

POSITION SUMMARY

The Navigator is a full-time position responsible for the client management and coordination of resources for homeowners and community members utilizing NeighborWorks Northeastern Pennsylvania (NWNEPA) services.  In conjunction with program directors and staff in the Aging in Place program, Homeownership Sustainability program and Neighborhood Revitalization program, the Navigator will provide direct services to homeowners, prospective homeowners, and community members with the goal of assisting clients access NWNEPA and community services. 

PRIMARY RESPONSIBILITIES

Provide application and document retrieval assistance and resource connection to older homeowners and community members in applying for benefits and resources that may make their homes safer and more affordable.

Provide support to all programs through client engagement. 

Provide support to Friendly Visitor’s/CARE callers. 

Conduct follow up calls to and for older adults requesting information regarding NWNEPA Aging in Place services.

Connect incoming clients to appropriate NWNEPA services.

Maintain connections with other social service organizations to ensure the most up to date resources are provided and referrals are followed up on in a timely manner.

Attend meetings as assigned by NeighborWorks NEPA. 

Maintain clear and concise records, including timely data entry in data management system. 

Work with Aging in Place Coordinator, Home Sustainability Program Manager and Neighborhood Revitalization Manager on reporting, meeting coordination, and other tasks.

Uphold high organizational standards. Performs related duties or responsibilities, as assigned, or requested.

EDUCATION & EXPERIENCE

A Bachelor’s degree and specific experience working in social services and/or education in social services is preferred.  

JOB REQUIREMENTS / SKILLS

1.     Excellent communication and customer service skills.

2.     High level of diplomacy, unquestionable integrity, and the demonstrated ability to deal confidentially with sensitive situations and information.

3.     Strong organizational and time management skills.

4.     Proficient in Microsoft Office Suite (Word, Excel and PowerPoint) and internet navigation.
